Date/Time: Select the date and time for your appointment. A drop-down list will show all the available timeslots for the next 7 days. Since this service is world-wide, it is important to know that there might be a time zone difference between you and the caller. Please note that all timeslots you see online is shown in US Eastern Time.

Report No.: Please enter your 9 digits Mycometrics report number which is located on the upper left corner of first page of your report (for example: 211138-015). If you have more than one ERMI report, you may reserve another timeslot by entering your second report number. Please have your ERMI report(s) ready at the time of your appointment.

Phone No.: Please enter your phone number(s) where you can be reached for this appointment.

Email Address: We will send you a confirmation via email. Please make sure the spellings are correct.

Comments: Any information you would like to share with our tech associate, you may mention in there.

Confirmation: Once your online request is received, we will send an email confirming your appointment. If you don't receive your confirmation email, feel free to call our customer service.

Remarks: If you miss our associate's call, you can call us back within the remaining time of your appointment. Keep in mind that your time will be reduced by the amount of time you were delayed to accommodate the next client's scheduled appointment.

We understand that emergencies and other scheduling conflicts arise and are sometimes unavoidable. However, advance notice allows us to fulfill other customer's scheduling needs and keeps our office operating at its most efficient level. This policy is in place out of respect for our associates and customers. Any appointment missed, late cancelled or changed without 24-hour notice are difficult to fill. By giving last minute notice or no notice at all, you prevent someone else from being able to schedule into that timeslot, and leave a 30-minute hole in our associates' schedule. Please provide our office with 24-hour notice to change or cancel an appointment. Customers who do not attend a scheduled appointment or do not provide 24-hour notice to change a scheduled appointment is responsible for a $30.00 no-show fee. This fee must be paid online before the next scheduled appointment.
